The information that you supply to Staffordshire County Council (Data Controller) will be used solely for the purposes of the Dignity in Care Awards.

The information which you provide as part of the nomination, relating to you and the nominated person, will be shared with other agencies outside of the Council.   A Judging Panel made up of representatives from the Dignity in Care Council, Staffordshire County Council and MPFT (Midlands Partnership Foundation Trust) will have access to the information as part of the judging process; as well as our chosen presenter(s) and our chosen partner assisting with production of the digital content.

As part of this process we may publish details of both you and the person/organisation you nominate within the Awards Programme, on the Staffordshire County Council’s website, on posters, in newspaper articles and other social media platforms. We will obtain their consent before we do this.

When the nominations have been received, we will write to those individuals who have been nominated and inform them that they have been nominated, which category they have been nominated for and the person who nominated them.

Where your nomination relates to a young person, we would strongly advise that you seek parental consent prior to making the nomination.
