The information you supply will be used solely for the purpose of the Dignity in Care Awards - by making your nomination, please be advised that your nomination (including who you are nominating) will be shared with others.  As part of this process the information you provide will be seen by a Judging Panel (the Panel will be made up of representatives from Dignity in Care, Staffordshire County Council and Clinical Commissioning Groups.)  As part of this process we may wish to publish details of both you and the person/organisation you nominate within the Awards programme; for any young person who is being nominated we advise you to seek parental consent prior to nomination, because the information may be in the public domain.
