Overview

'Feedback Fortnight' is taking place between 29th November and Friday 10th December. It's an opportunity to provide feedback, views and opinions on Staffordshire's Children & Family Services. There are only 3 questions to answer about the service you are providing feedback about.

We welcome feedback from young people, parents/carers, families and professionals on any of our services. If you would like to give detailed feedback about the SEND (special educational needs and disabilities) service we invite you to complete the SEND survey instead of this one which only contains overview questions. You can find that survey here

We appreciate you taking the time to fill out this survey and as a thank you are offering a prize draw. You will be asked if you would like to enter once you have submitted your survey.

What happens next
